How We Use Your Information Privacy Notice
Sharing your information to improve care
We are making sure that whether you are visiting a GP, attending hospital, or being seen in the community or at home by a care professional, that everyone knows the care you need and how you want to be treated.
The Benefits
You can talk to our health and care professional about sharing your information but here are some of the benefits:
- You only need to tell your history once
- You avoid unnecessary appointments and tests
- You can be more involved in decisions about your care
Your Choice
You can choose whether to share your information and who can see it. If you do not want to share, please tell your GP or complete either of the forms below, returning them to the practice so that your record can be updated.

Our practice works in partnership with the NIHR Clinical Research Network (CRN)

Summary Care Records
If you are registered with a GP practice in England, you will have a Summary Care Record (SCR), unless you have previously chosen not to have one. It includes important information about your health:
Medicines you are taking
Allergies you suffer from
Any bad reactions to medicines.
You may need to be treated by health and care professionals who do not know your medical history. Essential details about your healthcare can be difficult to remember, particularly when you are unwell or have complex care needs. Having an SCR means that when you need healthcare, you can be helped to recall vital information.
SCRs can help the staff involved in your care to make better and safer decisions about how best to treat you.
You can choose to have additional information included in your SCR, which can enhance the care you receive. This information includes:
Your illnesses and health problems
Operations and vaccinations you have had in the past
How you would like to be treated - such as where you would prefer to receive care
What support you might need
Who should be contacted for more information about you.

Personal Information
We are fully computerised. We are registered under the Data Protection Act and have robust systems in place to protect your confidentiality. Personal health information is used to monitor our screening activities. Anonymised health information is sent to the PCT to monitor quality standards and for payment verification purposes.
Please inform us of any changes to your personal details as soon as possible to keep our records up -to-date. Under the Data Protection Act you may view your records, speak to a doctor for more information and charges
Freedom of Information
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme which is a guide to the ‘classes’ of information the practice intends to routinely make available - contact the practice administrator for details.
